We are asking that participants use the same location for the final screening as they did for initial screen. Your personal Health Screen Record will be given to the nurse at the initial location. Those who originally weighed-in at Tutu’s House, Five Mountains office or Parker School are invited to go to any of 3 Waimea locations for final screens (Kaiser, NHCH or Tutu’s) and your records will be at each. Others may go to different final screen locations and nurses will forward info to FMH to match with original record. Don’t forget to bring your Personal Health Log book with you so you can record your progress for yourself. For health screen questions, please call Sally Kaufmann, RN at 345-2799.
All who complete the GFH program will receive, at the health screen, a coupon for a great free gift – a glass water bottle with a labels of positive words to customize your own bottle. This item was selected by our Team Leaders at Orientation. Thank you to Holly and Eila Algood for designing the custom positive thought labels! You may redeem the coupon & pick up your gifts at the November 5th event at Anna’s Ranch (11:30 – 2:30) or at the Five Mountains office in Waimea anytime in November.

A totally fun and healthy event with scavenger hunt, poker walk, zumba sessions, a 1.5 mile walk around downtown Honokaʻa, and potluck. Prizes will be awarded for Best Poker Hand, Worse Poker Hand, Best Hunt, etc.
